Question

68-year-old patient complaining of left wrist pain on thumb extension.
(no obvious trauma)

Ultrasound: Fluid layer in the sheath of the extensor pollicis brevis and abductor pollicis longus tendons. No intra-articular effusion or edema of the cutaneous and subcutaneous soft tissues.
No hypervascularization on Doppler.

What next steps should be taken?

ANSWER

This is a De Quervain's tenosynovitis warranting an ultrasound-guided injection
